Dear Patient,
With mixed emotions, I wish to inform you that I will be closing my behavioural pediatric practice effective February 28, 2023.
As a consultant, my operating practice has been to work closely with your primary health care provider. This means that I would regularly provide them with all consultation and progress notes detailing your diagnosis, recommendations, treatment plans, and any anticipatory guidance needed. Your referring provider also received a copy of any laboratory investigations ordered.
Please contact your primary care provider first for any medical records. In the unlikely event that my office has the only copy of a record, contact us at (519) 979-9988 for further instructions. When my practice closes, this website will be the best place to request any of your medical records.
It has truly been a privilege engaging with children and families over my 30 years of practice in Windsor-Essex County. I have learned so much from you and you have enriched my life in so many ways. I will surely miss my patients and their families. In my new role, I will continue to advocate for the best health care for every child in our region—and in Canada.
